Summer office hours are
8 am ~ 3 pm Monday through Thursday.
The office will be closed June 30 through
July 4, and Fridays through August 8.

Discipline Agreement ~ required for every student

Health Record ~ immunization record must be on file for every student
~ physical exam required for K5 and grade 3

Sports Physical/Athletic Release
                            ~ All students in grades 6 and 9 and all student athletes need a sports
                                      physical instead of the regular physical.
                                   ~ Fill in the medical history portion of the form and take it with you to
                                      your appointment.
                                   ~ All athletes must also have a signed Athletic Release.
                                   ~ Sports physicals are available for $15 with Dr. Thomas Melham, 287-8596.
                                      Be sure to mention that you are a Heritage Hall student.
                                   ~ Central Indiana Orthopedics will also offer $15 phyicals on Saturday,
                                      June 14, 2008, only, from 8:00-11:00 am at 3600 W. Bethel.
